UCA reduces the overall cost of products and
services by linking our members to groups of consumers and by
negotiating volume discounts with vendors and providers on
behalf of our subscribers.

Is UCA an
insurance program?
UCA is not insurance. We offer a savings program
that discounts costs on products and services. Among our many
services, certain of our programs discount healthcare costs
without requiring medical histories, physicals, underwriting,
waiting periods or usage limits. And UCA membership includes
all members of the cardholder's household at no extra cost.

Who can
apply for a UCA card?
Everyone is eligible to sign up.

Can any
member of my family use my UCA card, or do I need to purchase
a separate card for each family member?
One UCA card provides savings for your entire
household, including your extended family if they reside at
the same address.

How long
will it take to receive my UCA card?
You should receive your UCA card within 2 weeks of
the date we receive the application.

When will
my UCA card be effective?
Your UCA card is effective upon receipt, usually
within 2 weeks of the date you submit your application.

I submitted
an application for a UCA card over three weeks ago, but I
haven't received it in the mail. What should I do?
Contact your representative to request information
on the status of your application.

Can groups
(e.g. associations, companies) sign-up for UCA services? Do
groups have any unique privileges in the UCA network?
Reduced membership fees may be negotiated for large groups of
UCA members. Contact a UCA marketing representative for
details.

Why would a
medical provider participate in the UCA program?
UCA offers providers the benefit of new
clients, the ability to better serve the community and
reduced advertising and marketing costs. Membership in the
UCA network is mutually beneficial to the provider and the
UCA member.

Will my
doctor or dentist be familiar with the UCA medical discount
program?
Your doctor will be familiar with the names of the
national networks imprinted on our membership cards. If your
doctor has questions, simply have him or her contact Customer
Service at the number listed on your card.

Are
discounts the same at every physician, dentist, clinic,
hospital, etc. in the UCA network?
No, they are not. Costs vary depending on the
provider, the service provided and the geographic location
where the service is rendered.

What
medications can be ordered through UCA’s Mail Order Service?
You can order maintenance medications for which
you have a prescription. You will save more if you have your
doctor write your prescription for at least a three-month
supply.

How do I
locate a list of health care providers in my area?
You may use the Provider Locator on our website to
find a participating provider in your area. Enter your Zip
Code and choose a category of medical service from the
drop-down menu to view a list of providers that meet your
criteria.

What if I
need to visit a doctor or have a prescription filled while I
am on vacation?
Simply present your UCA card at a participating
provider to receive UCA discounts, just as you would at home.
We suggest that you use the Provider Locator in advance to
locate providers in the area you are planning to visit.

How do I
take advantage of the UCA discounts?
After you have received your UCA membership card,
simply present it to participating providers. Your card
identifies you as a network member, and you will
automatically receive the discounts that have been negotiated
for you.

When do I
receive the discount on medical services and products?
UCA provides an immediate discount on medical
services and products. No need to fill out additional forms
or wait for reimbursement. Simply pay the provider the
discounted rate.

What is the
advantage of using the UCA program in conjunction with
insurance or Medicare?
UCA offers savings on services and products that
may not be covered by insurance or Medicare (e.g. dental,
pharmacy, vision, elective surgery, glasses, etc.) and UCA
does not impose restrictions for age or medical conditions.
In addition, UCA provides a choice of providers.

Are there
any restrictions imposed for medical conditions?
Unlike insurance, there are no restrictions on
access to medical services due to health conditions. UCA
welcomes all persons, even those with uninsurable illnesses
like terminal cancer, AIDS, muscular dystrophy and other
catastrophic illnesses.

Are there
any age restrictions on the UCA program?
There are no age restrictions on the UCA program.
In fact, UCA also has many specialty services for newborn
babies, children, and elderly persons.

Is there a
limit to the number of times I may visit a UCA provider?
There is no limit to the number of times you may
visit a UCA health care provider. Use the card as often as
possible to maximize your savings.

Whom should
I contact if I move and need to update my address in your
records?
You may update your address by contacting your
representative or calling the customer service number on your
card.

What if I
am dissatisfied with the UCA card?
We hope that you will be completely satisfied with
the UCA benefit program, but if you decide it is not for you,
simply call our toll-free customer service # to cancel at any
time. If you return your membership kit and cards within 60
days of purchase you will receive a full refund of the cost
of the program (less the application processing fee).
